To move from field to field, you can use your tab key or mouse to the next colored area.
 

Use the built-in print button at the top of the form; it will take a few extra moments but will adjust your printer settings so that the form prints properly.
 

Save a blank copy of the form on your hard drive to use as a template.
 

Submit your completed form to us by facsimile.
